This program is an adaptation of the evidence-based, Stanford University developed Chronic Disease Self-Management Program. Participants of the workshop are taught self-management tools that empower them to address their health issues and live overall healthier lives.
Workshop Format:
  • 55-minute sessions, twice a week, for six weeks
  • Facilitated by two certified leaders
  • 12-14 employees per workshop
